Continuum of Care (CoC)
What is the York City/County Continuum of Care (CoC)?
A Continuum of Care (CoC) is a local or regional planning body that coordinates housing and services funding for homeless families and individuals. In 1995, the Department of Housing and Urban Development (HUD) instructed communities that they had to apply for federal homeless funding through a coordinated process – and the CoC was born.
From 1996 to 2004, York City & County operated a local, county-wide CoC Committee and HUD application process, however, it was determined that additional dollars may be available for our community if York joined the Central/Altoona Regional Housing Advisory Board – a regional group of counties in central Pennsylvania that would apply for HUD funding together.
From 2005 to 2010, York participated in this regional planning process. In 2010, it was determined that York should again bring their planning and application process back to a more local, county-wide practice and so in 2011 the York City & County Continuum of Care was brought back.
York County Planning Commission serves as the government liaison (or CoC Lead Agency) for the County of York in the HUD application process and the CoC Committee, and plays an active role in determining plans to prevent and end homelessness in York County.
The Continuum of Care (CoC) Committee
The York City & County CoC Committee was created in 2011 with HUD approval. The York County Planning Commission (YCPC), CoC Lead Agency, led the effort to form the CoC and approached the York Housing Advisory Commission (YHAC) to oversee the process. The CoC became a Committee of the YHAC. The YCPC worked with United Way of York County and Adams-Hanover Counseling to identify key CoC agencies. Participants range from those who work in homeless serving housing or service projects, to health care, government, and other interested parties.

The Continuum of Care (CoC) Funding Process
The Continuum of Care funding process is directed through the CoC Committee, listed above, as well as the York County Planning Commission (YCPC). The funding process is dependent upon open and close dates for applications that are produced by the Department of Housing and Urban Development (HUD). Typically, the application process occurs in the Fall, with funding being awarded the following Spring. Projects must meet the specifications of housing and project types set forth by HUD through the HEARTH Act, as well as local specialized priorities which are set by the CoC Committee. Projects that apply for this funding stream must be ranked and approved by a sub-committee of the CoC Committee.

As the local planning body for housing and services funding for homeless families and individuals, the CoC is also responsible for creating and implementing a 10 Year Plan to End Homelessness. This plan provides a strategic, 10-year vision for eliminating homelessness in those currently experiencing it, as well as preventing new individuals and families from becoming homeless.
